How It Works
Carvedilol blocks beta receptors in the heart and circulation and also blocks alpha-1 receptors in blood vessels. Beta blockade slows the heart’s response to stress hormones, while alpha-1 blockade helps relax blood vessels. Together, these effects can reduce blood pressure, decrease cardiac workload, and improve the efficiency of the heart in selected cardiovascular conditions.
Scientific reviews also describe additional pharmacological properties, including vasodilatory effects and antioxidant activity, although the main clinically established actions are adrenergic receptor blockade and blood-pressure reduction.
Important Safety Information
Carvedilol should not be used in patients with bronchial asthma or related bronchospastic conditions, severe bradycardia, second- or third-degree atrioventricular block, sick sinus syndrome, cardiogenic shock, decompensated heart failure requiring intravenous inotropic therapy, severe hepatic impairment, or a serious hypersensitivity reaction to carvedilol or related products.
Important precautions include the risk of bradycardia, hypotension, fluid retention or worsening heart failure, and bronchospasm in susceptible patients. Abrupt discontinuation can worsen coronary artery disease or angina symptoms, so therapy changes require clinical supervision.
Clinically significant interactions may occur with other medicines that lower heart rate or blood pressure, including certain calcium channel blockers, digoxin, amiodarone, clonidine, reserpine, and monoamine oxidase inhibitors. CYP2D6 inhibitors may increase carvedilol exposure, while rifampin may reduce it. Carvedilol may also affect glucose control and can mask some warning signs of low blood sugar.
Possible Side Effects
Common adverse effects reported in prescribing information include dizziness, fatigue, hypotension, diarrhea, bradycardia, asthenia, weight increase, and, in some settings, hyperglycemia. Dizziness is a frequent complaint in treatment for hypertension.
More serious reactions can include marked slowing of the pulse, fainting, worsening heart failure, severe low blood pressure, bronchospasm, allergic reactions, and liver-related problems in susceptible individuals.

Clinical reviews and trials support carvedilol as an effective therapy in chronic heart failure, where it has been associated with improvements in left ventricular function and reductions in morbidity in studied populations.
