Active Ingredient and Medicine Class
Clomiphene citrate is a selective estrogen receptor modulator with anti-estrogenic effects in some tissues and estrogen-like effects in others. In reproductive medicine, it is used as an ovulation-inducing agent. The tablet is taken orally and contains clomiphene citrate as the active ingredient.
Established Medical Uses
Clomiphene citrate is indicated for the treatment of ovulatory dysfunction in women who desire pregnancy, when other causes of infertility have been excluded or appropriately treated. Official prescribing information describes use in anovulatory patients, including some patients with polycystic ovary syndrome and other forms of ovulatory dysfunction.
It is not recommended for use in pregnancy. It is also not intended to replace specialist assessment of infertility causes or to treat every type of infertility.
How It Works
Clomiphene citrate acts by binding to estrogen receptors, especially in the hypothalamus and pituitary. By reducing the brain’s normal estrogen feedback signal, it increases the release of gonadotropin hormones, which can stimulate follicle development and trigger ovulation. It may also interact with estrogen-responsive tissues in the endometrium, cervix, and vagina.
Important Safety Information
Clomiphene citrate is contraindicated in pregnancy, in patients with hypersensitivity to the medicine or its ingredients, and in people with liver disease or a history of liver dysfunction. Prescribing information also lists contraindications in patients with ovarian cysts or enlargement not due to polycystic ovary syndrome, abnormal uterine bleeding of unknown cause, uncontrolled thyroid or adrenal dysfunction, and organic intracranial lesions such as pituitary tumours.
Important precautions include the risk of ovarian enlargement and ovarian hyperstimulation syndrome, which can become serious. Visual disturbances are a recognised concern and may be prolonged or, rarely, irreversible. Caution is also warranted in patients with hyperlipidemia because hypertriglyceridemia has been reported. Clinically significant interactions are not well documented in the official label, so concurrent medicines should still be reviewed carefully in routine prescribing practice.
Possible Side Effects
Common adverse effects include ovarian enlargement, vasomotor flushes, abdominal or pelvic discomfort, bloating, nausea, vomiting, and breast discomfort. Visual symptoms such as blurring, spots, or flashes are important adverse effects that require attention.
Serious reactions reported in prescribing information include ovarian hyperstimulation syndrome, marked ovarian enlargement, thromboembolic events, pancreatitis, and rare severe or persistent visual problems. Multiple pregnancy is an important treatment-related risk when conception occurs.
Scientific Evidence
Clinical research and reviews support clomiphene citrate as an effective ovulation-induction medicine, particularly for anovulatory infertility and polycystic ovary syndrome. Human studies show that it can increase ovulation rates, although pregnancy and live-birth outcomes are not identical to ovulation outcomes, and treatment response varies between patients.
Published reviews also note that clomiphene’s anti-estrogenic effects may reduce endometrial receptivity or alter cervical mucus in some women, which may help explain why ovulation does not always translate into pregnancy. Multiple pregnancy and ovarian hyperstimulation remain the main clinically relevant safety concerns in the fertility setting.
