Jpleprill 10 mg is a tablet medicine containing enalapril maleate, an angiotensin-converting enzyme (ACE) inhibitor. Enalapril is an established cardiovascular medicine used mainly to lower blood pressure and, in selected patients, to support treatment of heart failure and left ventricular dysfunction.

How It Works

Enalapril lowers the activity of ACE, an enzyme involved in the renin-angiotensin-aldosterone system. This reduces formation of angiotensin II, a substance that narrows blood vessels and promotes sodium and water retention. The result is less vascular resistance and reduced workload on the heart. Enalaprilat, the active metabolite, is responsible for the main pharmacological effect.

Important Safety Information

Enalapril should not be used in patients with a history of angioedema related to previous ACE inhibitor treatment. It is contraindicated in pregnancy because medicines that act on the renin-angiotensin system can cause fetal harm. Co-administration with aliskiren is contraindicated in patients with diabetes, and dual blockade of the renin-angiotensin system can increase the risk of hypotension, kidney dysfunction and high potassium levels.

Important precautions include reduced kidney function, dehydration or salt depletion, prior angioedema, and use with medicines that may raise potassium or affect renal function. Clinically significant interactions can occur with potassium-sparing diuretics, potassium supplements, NSAIDs, lithium, and certain immunosuppressive or mTOR-inhibitor therapies. Blood pressure, kidney function and serum potassium may be affected during therapy.

Possible Side Effects

Commonly reported adverse effects include dizziness, low blood pressure, cough, headache and fatigue. Some patients may experience changes in kidney function or elevated potassium.

Serious but less common reactions include angioedema, severe hypotension, worsening renal function, hyperkalemia and rare liver-related adverse effects. Any swelling of the face, lips, tongue or throat is medically urgent.

Scientific Evidence

Human clinical and review literature supports enalapril as an effective ACE inhibitor that reduces blood pressure without a compensatory rise in heart rate. Reviews also describe benefit in heart failure, including improvement in symptoms and haemodynamic measures in selected patients. More recent review evidence in children notes that enalapril is used in paediatric hypertension, heart failure and chronic kidney disease, although the evidence base is less complete in younger children and in some heart-failure settings.
