Pantosec-D
Pantosec-D is a combination medicine containing pantoprazole and domperidone. It brings together an acid-suppressing proton pump inhibitor and a prokinetic antiemetic agent. Verified regulatory information for the individual ingredients supports their use in acid-related upper gastrointestinal disorders, while domperidone carries important cardiac safety restrictions.
Active Ingredient and Medicine Class
Pantoprazole is a proton pump inhibitor (PPI). It reduces gastric acid production by blocking the gastric proton pump in the stomach lining. Domperidone is a dopamine antagonist with prokinetic and antiemetic effects.
Established Medical Uses
Pantoprazole is established for the treatment of gastro-oesophageal reflux disease and erosive esophagitis, and for maintenance of healed erosive disease. Domperidone’s approved benefit-risk position is best supported for relief of nausea and vomiting; EMA found that evidence was not sufficient to support other indications. Combination use with a PPI has been studied in gastro-oesophageal reflux disease and related symptom control.
How It Works
Pantoprazole lowers stomach acid, which can reduce acid exposure to the oesophagus and help heal acid-related injury. Domperidone blocks dopamine receptors in the gut and the chemoreceptor trigger zone, which can help improve upper gastrointestinal motility and reduce nausea-related symptoms.
Important Safety Information
Pantoprazole is contraindicated in people with hypersensitivity to the drug or other proton pump inhibitors. Important warnings include the possibility of acute interstitial nephritis, Clostridioides difficile-associated diarrhoea, hypomagnesaemia, fracture risk with long-term use, and the need to consider gastric malignancy if symptoms persist. It may interact with methotrexate, and long-term acid suppression can affect vitamin B12 status.
Domperidone has a recognised risk of serious cardiac adverse reactions, including QTc prolongation, torsade de pointes, ventricular arrhythmia and sudden cardiac death. Risk is higher in older adults, at higher oral doses, and with concomitant QT-prolonging medicines or CYP3A4 inhibitors. It is contraindicated in patients with significant cardiac conduction disorders, underlying cardiac disease such as congestive heart failure, severe hepatic impairment, and with certain interacting medicines.
Possible Side Effects
Reported adverse effects of pantoprazole include headache, diarrhoea, nausea, abdominal pain and gas. More serious reactions can include allergic reactions, kidney inflammation, low magnesium, severe diarrhoea, and fractures with prolonged therapy. Domperidone can cause dry mouth and abdominal discomfort, and rarely but importantly can cause serious rhythm disturbances and sudden cardiac events.
Scientific Evidence
Clinical research and a systematic review have examined domperidone used with proton pump inhibitors in gastro-oesophageal reflux disease. The evidence suggests possible symptom benefit in selected patients, but the overall evidence base is limited and must be balanced against domperidone’s cardiac safety concerns. Pantoprazole itself has robust evidence for reflux disease and erosive esophagitis in placebo- and comparator-controlled studies.
