How It Works
Avanafil helps relax smooth muscle and increase blood flow to the penis during sexual stimulation, which can improve the ability to achieve and maintain an erection. The combination is intended to address two different sexual function concerns at the same time.
Important Safety Information
Avanafil should not be used with nitrates because of the risk of a marked drop in blood pressure. It is also contraindicated with guanylate cyclase stimulators such as riociguat or vericiguat. Caution is needed in people with cardiovascular disease, in those taking alpha-blockers or other blood-pressure-lowering medicines, and with strong CYP3A4 inhibitors. Severe liver impairment and severe kidney impairment are important restrictions in the prescribing information.
Dapoxetine has important interaction risks with other serotonergic medicines, including monoamine oxidase inhibitors and other antidepressants that increase serotonin activity, because of the risk of serotonin syndrome. It is also avoided in people with significant heart rhythm problems, a history of fainting, or moderate to severe liver impairment. Possible Side Effects
Commonly reported adverse effects with avanafil include headache, flushing, nasal congestion, nasopharyngitis and back pain. Important but less common reactions include prolonged erection, sudden vision changes, sudden hearing loss and low blood pressure.
With dapoxetine, common side effects include nausea, dizziness, headache, diarrhoea and sleep-related or postural symptoms. More serious concerns include fainting, mood changes, serotonin syndrome when combined with other serotonergic medicines, and cardiovascular reactions in susceptible patients.

Clinical trial evidence supports avanafil as an effective PDE5 inhibitor for erectile dysfunction, including improvement in erectile function and rapid onset in some studies. Dapoxetine has also shown benefit in placebo-controlled trials for premature ejaculation, improving ejaculatory latency and patient-reported control.
For men with both premature ejaculation and erectile dysfunction, research has also examined dapoxetine used alongside PDE5 inhibitor therapy, showing improved ejaculatory outcomes with acceptable short-term tolerability in selected study populations.
