Active Ingredient and Medicine Class
Voriconazole is a broad-spectrum triazole antifungal. It belongs to the azole class of medicines that act against fungal pathogens by inhibiting a key enzyme needed for fungal cell membrane synthesis.
Established Medical Uses
According to U.S. prescribing information, voriconazole tablets are indicated for the treatment of invasive aspergillosis, candidemia in non-neutropenic patients and other deep tissue Candida infections, esophageal candidiasis, and serious infections caused by Scedosporium apiospermum and Fusarium species when other therapy is not suitable or has failed.
How It Works
Voriconazole blocks the fungal enzyme 14-alpha-sterol demethylase, which interferes with production of ergosterol, an essential component of the fungal cell membrane. This weakens fungal growth and helps control the infection.
Important Safety Information
Voriconazole should not be used in people with known hypersensitivity to the medicine or its excipients. It is also contraindicated with certain medicines that are highly dependent on CYP3A4 metabolism or with strong enzyme inducers that can markedly reduce voriconazole exposure.
Important warnings include hepatotoxicity, QT prolongation and arrhythmias, visual disturbances, severe skin reactions, photosensitivity, adrenal dysfunction, embryo-fetal toxicity, and skeletal adverse reactions with long-term therapy. Voriconazole has many clinically significant drug interactions, so concomitant medicines require careful review.
Possible Side Effects
Common adverse reactions reported in adults include visual disturbances, nausea, vomiting, rash, headache, fever, chills, abnormal liver tests, tachycardia, and hallucinations. More serious reactions can include liver injury, severe skin reactions, photosensitivity, and neurotoxicity.
Scientific Evidence
Clinical reviews and prescribing information support voriconazole as an effective treatment option for invasive aspergillosis and selected Candida and rare mould infections. Published reviews also note that oral voriconazole has good absorption, but its use is limited in some patients by frequent drug interactions and the need to monitor for hepatic and neurologic adverse effects.
