Sildenafil and tadalafil are established treatments for erectile dysfunction in adult men. Tadalafil is also approved in some products for benign prostatic hyperplasia or pulmonary arterial hypertension, depending on formulation, while sildenafil is also approved in some formulations for pulmonary arterial hypertension. How It Works
PDE5 inhibitors help preserve cGMP in penile tissue, which supports relaxation of smooth muscle and increased blood flow during sexual stimulation. Sildenafil and tadalafil act through the same pathway; tadalafil generally has a longer duration of action than sildenafil.
Important Safety Information
Both ingredients are contraindicated with any form of organic nitrate because of the risk of marked blood-pressure lowering. Caution is also required with alpha-blockers, antihypertensives, riociguat, and substantial alcohol intake, as these may further increase hypotension risk. The class should also be used carefully in people with cardiovascular disease for whom sexual activity is inadvisable. Labeling also warns about hypersensitivity, priapism risk, and rare sudden vision or hearing problems. Combination use of one PDE5 inhibitor with another erectile dysfunction therapy has not been adequately studied.
Possible Side Effects
Common adverse effects reported with sildenafil and tadalafil include headache, flushing, dyspepsia, nasal congestion, back pain, and muscle pain. More serious reactions can include prolonged erection, low blood pressure, chest symptoms, sudden loss of vision, and sudden hearing changes.
Scientific Evidence
Peer-reviewed studies show that sildenafil and tadalafil are both effective for erectile dysfunction, with broadly similar overall efficacy and safety in comparative analyses. Some evidence suggests tadalafil may be preferred by many patients because of its longer window of effect, while adverse-effect patterns differ somewhat between the two medicines. However, the reviewed literature compares the individual drugs, not this fixed-dose combination as a standard regulated product.
