Description
Tadasiva
Tadasiva is a combination product identified by its stated contents of sildenafil and tadalafil. Both are phosphodiesterase type 5 (PDE5) inhibitors used in erectile dysfunction. Publicly available prescribing information supports the individual ingredients and their class effects, but no official source was found to verify Tadasiva as a distinct regulated trade name in the sources reviewed.
Active Ingredient and Medicine Class
Sildenafil and tadalafil are PDE5 inhibitors. This medicine class relaxes smooth muscle and increases blood flow in the penis in response to sexual stimulation. Sildenafil and tadalafil are established active ingredients in approved products for erectile dysfunction.
Established Medical Uses
The established use of sildenafil and tadalafil is treatment of erectile dysfunction. Tadalafil is also approved in some products for benign prostatic hyperplasia and pulmonary arterial hypertension, but those uses depend on the specific product and dose form. The reviewed evidence did not verify a unique approved indication for the fixed-dose combination Tadasiva itself.
How It Works
Both ingredients inhibit PDE5, the enzyme that breaks down cyclic guanosine monophosphate (cGMP). By preserving cGMP, they enhance nitric oxide–mediated vasodilation and help increase penile blood flow during sexual stimulation. Tadalafil generally has a longer duration of action than sildenafil, while sildenafil is typically associated with a shorter duration.
Important Safety Information
Known contraindications and major precautions for the ingredients include use with any form of organic nitrate because of the risk of marked blood pressure lowering. Tadalafil is also contraindicated with guanylate cyclase stimulators such as riociguat. Both ingredients warrant caution with alpha-blockers, antihypertensives, alcohol, and strong CYP3A4 inhibitors because of additive hypotension or increased drug exposure. Sildenafil and tadalafil should also be used cautiously in people with cardiovascular disease in whom sexual activity is not advisable.
Other important warnings include prolonged erection and priapism, sudden vision loss or NAION, and sudden hearing loss. Sildenafil prescribing information also notes a warning about use with other PDE5 inhibitors or other erectile dysfunction therapies.
Possible Side Effects
Common adverse effects of PDE5 inhibitors include headache, flushing, dyspepsia, nasal congestion, dizziness, and back pain. More serious but less common reactions include hypotension, priapism, sudden visual disturbance, sudden hearing loss, and allergic reactions.
Scientific Evidence
Direct clinical evidence for the fixed-dose sildenafil+tadalafil combination was not identified in the sources reviewed. Human studies show that sildenafil and tadalafil are both effective and generally well tolerated for erectile dysfunction when used separately, and review literature confirms that PDE5 inhibitors are first-line treatments for ED. Evidence for routine combined use remains limited, so product-specific efficacy and safety should not be assumed from the individual ingredients alone.
