How It Works
Tadalafil inhibits PDE5, helping preserve cGMP in penile tissue after sexual stimulation. This supports relaxation of smooth muscle and increased blood flow, which can help produce and maintain an erection. It does not create an erection without sexual stimulation.
Important Safety Information
Tadalafil is contraindicated with organic nitrates because of the risk of a marked drop in blood pressure. It is also contraindicated with guanylate cyclase stimulators such as riociguat. Important precautions include cardiovascular disease, low blood pressure, renal or hepatic impairment, and the risk of hypotension when combined with alpha-blockers, other antihypertensives, or substantial alcohol intake. Potent CYP3A4 inhibitors can increase tadalafil exposure.
Rare but serious risks include priapism, sudden vision loss linked to NAION, and sudden hearing loss. Patients with a prolonged erection require urgent medical attention.
Possible Side Effects
Common adverse effects include headache, dyspepsia/indigestion, back pain, myalgia, flushing, nasal congestion, and dizziness. Less common reactions reported in clinical studies include nausea, vomiting, arthralgia, and muscle spasm.
