How It Works
Sildenafil blocks PDE5 in the corpus cavernosum, which helps preserve cGMP and supports smooth-muscle relaxation. This makes erections more likely in response to sexual stimulation, but it does not create an erection on its own.
Important Safety Information
Sildenafil is contraindicated with nitrates and with guanylate cyclase stimulators because of the risk of marked hypotension. Caution is also required with alpha-blockers, antihypertensive medicines, and strong CYP3A4 inhibitors such as ritonavir, ketoconazole, and itraconazole, which can increase sildenafil exposure. Prescribing information also warns about cardiovascular risk in susceptible patients, prolonged erection/priapism, eye-related events, hearing loss, and possible bleeding concerns. Combining sildenafil with other PDE5 inhibitors or other erectile dysfunction therapies is not recommended.
Possible Side Effects
Common adverse effects include headache, flushing, dyspepsia, nasal congestion, dizziness, abnormal vision, back pain, myalgia, nausea, and rash. Important serious reactions can include priapism, severe low blood pressure, sudden vision loss, sudden hearing loss, and allergic reactions.

Systematic reviews and randomized trials found sildenafil to be more effective than placebo for improving erectile function and successful intercourse attempts, with common adverse effects generally being mild to moderate. A harms-focused review also found no clear signal of excess serious cardiovascular events in the trial data it analysed, though the product labeling still includes important cardiovascular warnings and contraindications.
