How It Works
Sildenafil works by inhibiting PDE5, an enzyme that breaks down cyclic GMP in the penis. This helps relax smooth muscle and increases blood flow into the erectile tissue during sexual arousal. The effect depends on sexual stimulation.
Important Safety Information
Sildenafil is contraindicated with nitrates and with guanylate cyclase stimulators such as riociguat because of the risk of a marked fall in blood pressure. It should also not be used in people with known hypersensitivity to sildenafil.
Important precautions include cardiovascular disease, since sexual activity may be unsafe for some patients with underlying heart conditions. Sildenafil may lower blood pressure further when used with alpha-blockers, antihypertensives, and alcohol. Strong CYP3A4 inhibitors, including ritonavir and some azole antifungals or macrolide antibiotics, can increase sildenafil exposure. Combination with other PDE5 inhibitors or other erectile dysfunction treatments is not recommended.
Possible Side Effects
Common adverse effects include headache, flushing, dyspepsia, abnormal vision, nasal congestion, back pain, myalgia, nausea, dizziness, and rash. Serious reactions can include prolonged erection or priapism, sudden vision changes, sudden hearing loss, symptomatic hypotension, and allergic reactions.

Peer-reviewed reviews and meta-analyses in PubMed support sildenafil as an effective PDE5 inhibitor for erectile dysfunction and describe a generally well-characterised safety profile. Clinical evidence also shows that adverse effects are usually dose-related and that blood-pressure interactions are clinically important, especially with nitrates and some vasodilators.
