How It Works
Sildenafil helps relax smooth muscle and widen blood vessels in the penis by inhibiting the PDE5 enzyme. This supports increased penile blood flow during sexual arousal. It does not cause an erection without sexual stimulation.
Important Safety Information
Sildenafil is contraindicated with organic nitrates and riociguat because of the risk of marked blood pressure lowering. It should also be used cautiously with alpha-blockers and other blood-pressure-lowering medicines because of additive hypotensive effects. Prescribing information warns that men for whom sexual activity is inadvisable because of cardiovascular status should not use sildenafil. Hypersensitivity to sildenafil or any tablet component is also a contraindication.
Possible Side Effects
Common adverse effects include headache, flushing, indigestion, nasal congestion, dizziness, back pain, myalgia, nausea and visual disturbance. Less common but serious reactions can include severe hypotension, chest pain, sudden vision loss, sudden hearing loss, prolonged erection and allergic reactions.
Scientific Evidence
Peer-reviewed reviews and meta-analyses show that sildenafil is more effective than placebo for improving erectile function and overall sexual performance in men with ED, while adverse effects are generally dose-related and usually mild to moderate. Safety concerns are greatest in patients taking nitrates or other medicines that substantially lower blood pressure.
