How It Works
Mirtazapine acts by increasing noradrenergic and serotonergic neurotransmission through presynaptic alpha-2 receptor antagonism. It also blocks certain serotonin receptors, especially 5-HT2 and 5-HT3, and has strong antihistamine activity at H1 receptors, which helps explain its sedating effect.
Important Safety Information
Mirtazapine is contraindicated in people taking, or who have recently taken, monoamine oxidase inhibitors, including linezolid and intravenous methylene blue. It should also be avoided in people with known hypersensitivity to mirtazapine or any tablet ingredient. Regulatory labeling warns about suicidal thoughts and behaviors in children, adolescents, and young adults, and it is not approved for pediatric use.
Important precautions include the risk of serotonin syndrome, angle-closure glaucoma, agranulocytosis, QT prolongation, DRESS and other severe skin reactions, mania or hypomania, seizures, hyponatremia, and elevated cholesterol or triglycerides. Clinically significant interactions may occur with strong CYP3A inhibitors or inducers, cimetidine, and warfarin. Caution is also advised in older adults and in people with renal or hepatic impairment.
Possible Side Effects
Common adverse effects include somnolence, increased appetite, weight gain, dry mouth, dizziness, and headache. More serious reactions can include severe skin reactions, blood count abnormalities, marked mood changes, and symptoms of serotonin syndrome.

Clinical reviews and meta-analyses support mirtazapine as an effective antidepressant for major depressive disorder. A recent systematic review found increased risks of somnolence, weight gain, dry mouth, dizziness, and increased appetite versus placebo, while evidence was insufficient to determine effects on suicides or serious adverse events over the short trial periods studied.
