Description

Rybelsus (Semaglutide) 3 mg

Rybelsus is a prescription medicine containing semaglutide, an oral glucagon-like peptide-1 (GLP-1) receptor agonist. It is used in adults with type 2 diabetes mellitus. The 3 mg strength is part of the oral formulation and is identified in regulatory information as a tablet presentation of semaglutide.

Active Ingredient and Medicine Class

The verified active ingredient is semaglutide. It belongs to the GLP-1 receptor agonist class. This class helps improve blood glucose control by enhancing insulin release in a glucose-dependent manner and reducing inappropriate glucagon secretion.

Established Medical Uses

According to the FDA prescribing information, Rybelsus is indicated as an adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellitus. The current FDA label also includes a cardiovascular risk-reduction indication for adults with type 2 diabetes at high risk of major adverse cardiovascular events.

How It Works

Semaglutide mimics the action of native GLP-1, a hormone involved in glucose regulation. In general terms, it helps the pancreas release more insulin when blood glucose is elevated and reduces glucagon secretion, which can lower liver glucose output. GLP-1 receptor agonists also slow gastric emptying and may reduce appetite. These effects support improved glycemic control.

Important Safety Information

Rybelsus is contraindicated in patients with a personal or family history of medullary thyroid carcinoma or in patients with multiple endocrine neoplasia syndrome type 2. The FDA label warns about acute pancreatitis, diabetic retinopathy complications, severe gastrointestinal adverse reactions, acute kidney injury related to volume depletion, hypersensitivity reactions, gallbladder disease, and hypoglycemia when used with insulin secretagogues or insulin.

Semaglutide may also affect absorption of other oral medicines because it slows gastric emptying. Patients with a history of diabetic retinopathy require particular caution, as rapid improvement in glucose control has been associated with temporary worsening of retinopathy in some settings.

Possible Side Effects

Common adverse effects reported with oral semaglutide include nausea, vomiting, diarrhea, abdominal pain, decreased appetite, and constipation. Important serious reactions include pancreatitis, hypersensitivity reactions, gallbladder problems, worsening diabetic retinopathy, and severe gastrointestinal intolerance. Low blood glucose is more likely when Rybelsus is combined with insulin or a sulfonylurea.

Scientific Evidence

In the PIONEER 6 cardiovascular outcomes trial, oral semaglutide was shown to be noninferior to placebo for cardiovascular safety in high-risk patients with type 2 diabetes. A later review of the trial results reported no excess in the primary composite cardiovascular outcome and supported the cardiovascular safety profile of oral semaglutide. Regulatory labeling now also includes cardiovascular risk reduction in selected adults with type 2 diabetes at high risk.
