Budesonide should not be used as the primary treatment for acute severe asthma symptoms or status asthmaticus. It is contraindicated in people with known hypersensitivity to budesonide or any component of the formulation. As with other inhaled corticosteroids, caution is needed when switching from oral or systemic corticosteroids, because adrenal suppression and adrenal insufficiency may occur during steroid withdrawal.
Clinically significant interactions may occur with strong CYP3A4 inhibitors such as ketoconazole, itraconazole, clarithromycin, ritonavir and related agents, which can increase systemic exposure to budesonide. Caution is also appropriate in patients with liver impairment, since corticosteroid exposure may be increased. Long-term high exposure may increase the risk of systemic corticosteroid effects.
Possible Side Effects
Common adverse effects of inhaled budesonide include throat irritation, hoarseness or voice change, cough, and oral candidiasis. Some patients may also experience headache or upper respiratory symptoms. More serious but less common reactions can include paradoxical bronchospasm, hypersensitivity reactions, adrenal suppression, reduced bone mineral density with prolonged corticosteroid exposure, cataracts, and glaucoma. Like other inhaled corticosteroids, budesonide can also be associated with immunosuppressive effects when used at higher exposure or for prolonged periods.
Scientific Evidence
Clinical trial reviews and systematic reviews support inhaled budesonide as an effective controller therapy for chronic asthma in adults and children. Published evidence shows improved lung function and asthma control compared with placebo, with reduced risk of exacerbations in persistent asthma. Review data also suggest that once-daily budesonide can be effective in selected patients with controlled or mild-to-moderate asthma, although the best regimen depends on the individual product and the severity of disease.
