Clinical studies supporting approval included adults and older adults with chronic insomnia, and also studies of transient insomnia in sleep-laboratory settings.
The medicine is used as a hypnotic rather than as a general treatment for anxiety, pain, or other sleep-related conditions. Any use outside the recognised indication should be guided by an appropriate clinician.
How It Works
Eszopiclone acts on the central nervous system by enhancing the effect of the inhibitory neurotransmitter GABA. Although the exact hypnotic mechanism is not fully defined, its action at GABA-receptor complexes near benzodiazepine receptor sites is thought to promote sleep by reducing neuronal activity and easing the transition into sleep.
Because it is a sedative-hypnotic, its effects can be influenced by other medicines or substances that also depress the central nervous system.
Important Safety Information
Eszopiclone has important safety restrictions. It is contraindicated in people who have previously experienced complex sleep behaviors with the medicine, such as sleep-walking or sleep-driving, and in people with known hypersensitivity to eszopiclone. Hypersensitivity reactions may include anaphylaxis and angioedema.
Clinically significant precautions include next-day impairment, especially with higher exposure, which may affect alertness, coordination, and activities requiring concentration. Caution is also advised in older or debilitated patients, in those with severe hepatic impairment, and when eszopiclone is used with other central nervous system depressants.
Important interactions include additive sedation with alcohol and other CNS depressants. Medicines that inhibit CYP3A4, such as potent inhibitors, can increase eszopiclone exposure, while CYP3A4 inducers can reduce its effect. Taking the medicine with a heavy, high-fat meal may delay absorption and reduce its sleep-onset effect.
Possible Side Effects
Commonly reported adverse effects include unpleasant taste, headache, somnolence, dizziness, dry mouth, anxiety, rash, respiratory infection, and hallucinations. Some reactions show a dose-related pattern in clinical studies, particularly unpleasant taste.
More serious reactions can include complex sleep behaviors, amnesia, abnormal behavior, mood or psychiatric changes, severe allergic reactions, and marked impairment of alertness. As with other hypnotics, withdrawal symptoms may occur if therapy is stopped abruptly after repeated use.
Scientific Evidence
Randomized trials and systematic reviews in adults with insomnia show that eszopiclone can improve sleep latency, wake after sleep onset, total sleep time, and subjective sleep quality compared with placebo. Longer studies also reported sustained improvements in sleep and daytime functioning over several months.
The clinical evidence supports its role as a sleep medicine, but benefits must be balanced against risks such as next-day impairment, complex sleep behaviors, and taste disturbance. Evidence is strongest for insomnia outcomes, not for broader health or daytime performance claims.
