How It Works
Mirtazapine works mainly by enhancing noradrenergic and serotonergic activity in the brain. It blocks central presynaptic alpha-2 receptors and also acts on certain serotonin receptors, which contributes to its antidepressant effect. It does not work by inhibiting serotonin reuptake.
Important Safety Information
Mirtazapine is contraindicated in people taking monoamine oxidase inhibitors, including linezolid and intravenous methylene blue, or in those with known hypersensitivity to mirtazapine or tablet excipients. Regulatory labeling also warns about suicidal thoughts and behaviors in children, adolescents, and young adults, and mirtazapine is not approved for pediatric use. Additional precautions include agranulocytosis, serotonin syndrome, angle-closure glaucoma, QT prolongation and torsades de pointes, DRESS, mania or hypomania, seizures, hyponatremia, and the potential for clinically relevant interactions with MAOIs, strong CYP3A4 inhibitors, and strong CYP3A inducers.
Possible Side Effects
Commonly reported adverse effects include somnolence, weight gain, increased appetite, dry mouth, dizziness, and headache. Serious reactions can include severe skin reactions, blood dyscrasias such as agranulocytosis, serotonin syndrome, and cardiac rhythm abnormalities. A recent systematic review found increased risks of somnolence, weight gain, dry mouth, dizziness, and increased appetite versus placebo, although long-term effects remain uncertain.
Scientific Evidence
Clinical reviews and comparative trials have found mirtazapine to be an effective antidepressant for major depression, with efficacy comparable to several established antidepressants in short-term studies. A 2024 systematic review of randomised trials found that mirtazapine increased several adverse effects versus placebo and that evidence on serious harms was limited.
