Active Ingredient and Medicine Class
Sildenafil is a phosphodiesterase type 5 (PDE5) inhibitor used for erectile dysfunction. Dapoxetine is a short-acting selective serotonin reuptake inhibitor (SSRI) developed for on-demand treatment of premature ejaculation. Together, they target two different sexual dysfunctions.
Established Medical Uses
Sildenafil is established for erectile dysfunction, where it helps improve the ability to achieve and maintain an erection in response to sexual stimulation. Dapoxetine has been studied and used for premature ejaculation, with clinical trials showing improvement in intravaginal ejaculatory latency time and patient-reported control and satisfaction. Combination use has been studied in men with both conditions.
How It Works
Sildenafil works by enhancing the nitric oxide/cGMP pathway in penile tissue, which relaxes smooth muscle and increases blood flow during sexual stimulation. Dapoxetine increases serotonin activity in the nervous system and is thought to delay ejaculation by modulating ejaculation reflex pathways.
Important Safety Information
Sildenafil is contraindicated with nitrates and should not be used with guanylate cyclase stimulators such as riociguat because of the risk of marked blood pressure lowering. Caution is also needed with alpha-blockers, antihypertensives, and strong CYP3A4 inhibitors such as ritonavir and some azole antifungals or macrolide antibiotics.
Dapoxetine should not be used with other serotonergic medicines, monoamine oxidase inhibitors, or strong CYP3A4 inhibitors because of the risk of serious adverse effects, including serotonin toxicity and increased dapoxetine exposure. Dapoxetine has also been associated with syncope and should be used cautiously in people with cardiovascular risk factors.
Possible Side Effects
Common effects reported with sildenafil include headache, flushing, dyspepsia, nasal congestion, dizziness, and visual disturbance. Dapoxetine commonly causes nausea, dizziness, headache, diarrhoea, and sometimes postural dizziness or fainting. In combination studies, these were the main adverse reactions observed; most were mild to moderate, but discontinuation sometimes occurred.
Scientific Evidence
Randomised and phase III studies show that dapoxetine improves ejaculation outcomes in premature ejaculation, including in men with concomitant erectile dysfunction treated with a PDE5 inhibitor. A clinical study of the dapoxetine/sildenafil combination reported improvements in ejaculation latency, erectile function scores, and patient satisfaction, with adverse events that were generally non-serious. Earlier pharmacokinetic work found no major interaction between dapoxetine and sildenafil, though clinical caution is still warranted because both medicines can contribute to dizziness or blood pressure effects.
